We are undertaking this health service improvement survey as part of our ongoing workplace improvement program. By providing us with your honest appraisal and ideas, we can identify areas for improvement and begin taking actions.

Confidentiality Statement

An external organisation, The Health Roundtable, is running the survey to guarantee your anonymity. All demographic questions are optional. If you provide demographic information, your responses will not be reported at an individually identifiable level. In practice, this means if there aren't five responses we won't show that column on the graph. If you provide any written comments, these will be reported with your professional grouping, length of service, and division if you provided these. Please report urgent issues through normal channels.
